Szacunki ofiar smiertelnych: 3787 oficjalnie, 8000-15000 wedlug raportow medycznych, do >25000 w dlugim terminie.

    Przyczyna zrodlowa: Awaryjne systemy zabezpieczen wylaczone z powodow ekonomicznych (chlodzenie tanku, scrubber NaOH, pochodnia spalajaca). Slabe szkolenie obslugi, nieprawidlowe procedury plukania rurociagow woda przy nieszczelnych zaworach izolujacych zbiornik MIC.
    Wnioski: Inherently safer design (Kletz): minimalizacja inwentarza substancji super-toksycznych. Layer of Protection Analysis (LOPA). Trojwarstwa redundantnych zabezpieczen MUSI byc operacyjna w kazdym momencie eksploatacji.
